To register at Whitefields Surgery will need to fill out a purple registration form, ethnic form and medical questionnaire which can be obtained from the receptionist, you MUST bring proof of address and an up to date Photo Identification. We will then call book you in to have a blood pressure appointment with the nurse. If you are on current medication please bring your repeat prescription list to the receptionist so the doctor can put these onto your records.
